Field Trips Permission Slips / Events / Volunteering

If your child's class will be going on a field trip, you will be notified directly by your child's teacher. We've included a field trip permission slip (generic blank) attached to this message in case you've misplaced the permission slip that was sent home with your child. Not all grades will have a field trip away from school. Some grades may have field trip-like programs held on campus where no travel is required.

If you are a parent, guardian or extended family member and wish to volunteer as a field trip chaperone, the State of California requires all field trip chaperones to be cleared at a Level 2. Please see the attachment that lists the requirements to be a Level 1 or Level 2 cleared volunteer. Level 2 LiveScan Background check forms can be picked up in the office or sent home with your child. Please contact the office to request the form. Keep in mind the Livescan fingerprinting and Department of Justice process can take two weeks or more to complete. Once the Livescan process is complete, it is valid for the entire time your child is enrolled in Fountain Valley School District. Livescans for other entities do not count, as they are confidential and go directly to our HR department once approved.

Sometimes events are held where family members are not required to have clearance, such as promotion activities, plays and other faculty supervised events where the family members present are not working directly with or supervising students, and rather attending a performance event.

If you are unsure, please contact the school office to determine whether volunteer clearance is needed for the specific activity or event being held.
